Home Care Packages are Government funded to assist older Australians live a safe and independent live in their own home. The funding is paid to your chosen Service Provider (like Bromilow Community Care) to arrange a Care Plan to meet your needs. There are 4 different levels of packages that can be assigned to meet your needs. Find out more here.

When My Aged Care has assigned you a Home Care Package and we have your referral code, one of our dedicated Care Managers will work with you, to develop a personalised care plan for you, that is within your budget to achieve your individual goals.  Our professional Care Managers work with you to ensure that your Care Plan moves with your changing needs.

CHSP helps older Australians access entry-level support services at a subsidised cost, to live independently and safely at home.  Support services can assist you with help at home, health services and keeping connected to your community . Often CHSP services are allocated prior to being assigned a Home Care Package.  Contact us for more information.

To get approved for a HCP you need to first register with the Government’s My Aged Care . If you prefer, you can phone us on 1300 158 242 and we can complete your referral and get you started.  The My Aged Care Assessment Team [ACAT] will look at various areas including your age and support needed. There are 4 levels of (HCP) which are assigned by the Aged Care Assessment Team (ACAT) according to your needs

  • Level 1: Basic care needs – approx. $9,000 per year
  • Level 2: Low care needs – approx. $15,750 per year
  • Level 3: Intermediate care needs – approx. $34,250 per year
  • Level 4: High care needs – approx. $52,000 per year

After you have been assessed by the Aged Care Assessment Team [ACAT] as being eligible, the wait time are as below.

  • Level 1: 3-6 months
  • Level 2: 12+ months
  • Level 3: 12+ months
  • Level 4: 12+ months

Yes, please contact our friendly team on 1300 158 242 and they will guide you through the process.  The process includes you contacting My Aged Care on 1800 200 422 and asking them to reactivate your referral code as you want to switch to Bromilow Community Care. We will then be able to organise a time for sign-up with you, so we can commence your care services.  We will also speak with your current provider to organise the date of transfer of your funds, so there is no gap in your services.  We organise everything from start to finish and it’s a really easy process for you to switch providers to us.

A Referral Code is a unique number that you receive when you are assigned a Home Care package.  You will find this number when you receive your letter from My Aged Care advising that you are assigned a Home Care Package.  This Referral Code allows your HCP Provider to access your ACAT assessment with details of your situation and care needs.

Bromilow Community Care ensure that you receive all of the supplements that you may be entitled to on top of your Home Care Package:

  • Dementia Supplement
  • Oxygen Supplement
  • Enteral Feeding Supplement
  • Veteran’s Supplement
  • Viability Supplement
  • Hardship Supplement

Unlike other providers we do not charge an entry or exit fee.
